Wearing your medical information is now fashionable when that info is on a stylish dog tag. This stainless steel tag is plated in a glossy black finish. The front and back can be engraved with your medical information. The engraving reveals the silver for a sleek look. A 18 inch ball chain is included. We also offer an upgraded neoprene necklace in black, silver or red. This tag is surgical stainless steel and hypoallergenic. It measures 1 1/4 x 3/4. The medical alert symbol is available in red or silver.

Engraving Information:

The front will be engraved vertically and can accomodate 5 lines of engraving with 12 maximum characters/spaces per line. The back can be engraved either vertically or horizontally. Vertically can accomodate 6 lines of with 12 maximum characters/spaces per line. Horizontally will handle 5 lines with 15 maximum characters/spaces per line.

Engraving on the front is included at no additional cost. Please specify engraving information in the text boxes below.

Due to the customization, please allow approximately two weeks to receive your tag.
